The Simple Way to Optimal Eye Health and Vision Care
The following is the HLEye Vision Plan that has been developed to assist various size businesses and organizations in our area to offer vision care to their members, employees and their family members at substantially reduced cost.
To utilize this program, you must be employed with an area business or organization whom wishes to participate in this program. The employer or organization official must verify employment or membership.

Hopewell Lambertville Eye Associates will offer our services at a substantial courtesy discount consistent with the vast number of vision plans on the market. This plan will also apply to immediate family members of the employee seen at our Hopewell or Lambertville offices. Your company will be supplied a Vision Care Card to be distributed to members of you group or your can simply print this page and have it signed by your employer. Simply present this card at the time of visit to appreciate the following discounts.

1. Reduction of normal annual exam fee from $145 to $75. (50% savings on eye health exam)
All examinations includes screening for glaucoma, retinal and cataracts. Dilated fundus exam is considered a medical procedure.and not part of the screening exam. If medical concerns, such as diabetes, glaucoma, cataracts, disease of retina or other medical findings, are found during the examination, additional care for the patient will be billed via their existing medical insurance. Emergency care is considered a medical concern and would be billed to the medical insurance carrier. If the individual has no medical insurance, they would be responsible for 60% of UCR charges.
2. A 30% courtesy on basic eyewear (CR39 lenses and frame) and 20% off on all options (limited to two pairs per year)
3. A 30% courtesy on contact lens professional fees. Maximum allowable discount $25 (basic) to $45 (complex) per fitting based on the complexity of the fitting. Fitting fees vary on complexity.
4. Savings of 15% on contact lenses: based on a maximum one year supply of disposable, frequent replacement contact lenses. Maximum allowable discount: $15 on a 6 month supply or $40 per year supply. (Rebates coupons are also available from the manufacturer’s usually ranging from $30 to $50 in addition to the 15% discount)
Estimated savings to an individual (not inclusive of Laser Vision Correction): NO cost to the employer or group. These estimates are consistent with present vision plans
Estimated savings per individual:
$70 exam per year // $230 to $ 530 eyewear and /or contacts
It is difficult to give an exact savings per patient due to their distinct individual needs.
